About the NDIS:

The NDIS is the largest social reform in Australia for decades. It is changing the lives of people living with a disability by giving them choice and control over the supports they access. Support Coordination plays an important role in helping some of the most marginalised members of society realise their full potential through the NDIS.

Locations

We are looking at hiring an experienced Support Coordinator to join our growing team in Melbourne, both the Northern Suburbs and the Eastern Suburbs.

We offer working from home and the office options which should meet the following criteria:

Training to be completed in the office (South Morang and/or Chadstone).
Working from Home flexibility when you have reached competency (approximately 2 months in the office).
Hybrid Model - 2 days at home and 3 days in the office.
The Role

A Support Coordinator is responsible for supporting the National Disability Insurance Scheme (NDIS) Participants to activate, interpret and implement their NDIS Plans. As a Support Coordinator, you will work with scheme participants between the ages of 0 – 65 with varying complexity levels. You will coordinate service providers and supports, liaise with government agencies, obtain quotes for services, organise service agreements and monitor Participants plans. You will also provide regular Support Coordination reports to the NDIA for each Participant you support.

Qualifications & Experience:

Minimum Certificate IV in Disability or related field.
Demonstrated knowledge and experience of NDIS.
Strong interpersonal, written, and verbal communication skills.
Ability to develop good working relationships with internal and external stakeholders, including participants, families, and health providers.
Effective time management, caseload management, problem-solving skills, and ability to plan and manage resources/budgets.
Knowledge and understanding of the NDIS, and ability to work within their framework.
Experience in working with people with complex support and service requirements.
Previous Support Coordination or Case Management experience.
Current First Aid and CPR Certificate (or willing to obtain).
Australian Driver's license and vehicle.
NDIS Worker Screening check (or willing to obtain).
Working with Children Check..
Tasks & Responsibilities:

Liaising and building rapport with key stakeholders such as: NDIS Providers and Agents, community venues, community partners, along with other revenue streams or allied health care teams.
Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
Independently manage a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
Conducting initial meetings with participants.
Researching and referring participants to relevant services when required with a focus on Recovery.
Ensure participants are working towards achieving their goals and being able to clearly document progress in reports
