Job Description

PHM Health is in the business of providing services and support to people with disabilities and assisting the community with independence. Our aim is to assist our participants in the pursuit of their life choices, either in their own home or the community, to enable greater quality of life experiences.

PHM Health provides support services including in-home assistance, accommodation support, and everyday life skill development both on a one-to-one basis and through group support options.

We are in search for enthusiastic and reliable Support Workers to support participants with complex behaviours.

The Role:

Our Support Worker looks after the wellbeing of participants in their daily lives. The Support Worker provides support and assistance to participants living with different physical disabilities and mental health needs to live their lives more independently and support them to reach their full potential by providing both physical and emotional support.

The main duties of a Support Worker include, but not limited to:

Assisting participants with daily living tasks and independent skills.

Provide personal care: personal hygiene and grooming.

Support with light domestic duties including meal preparations and cleaning.

Provide transport to appointments and/or facilitate community outings.

Arranging social activities with participants.

Delivering individual support plans.

Documenting services delivered, incidents and reports.

Skills & Requirements:

Mental Health experience is essential.

Excellent work ethic, have excellent oral communication and written communication skills, like a challenge, show initiative, enjoy working independently and as part of a team and think on your feet.

Fit, energetic, and enthusiastic individual who can see the potential in people.

Apply a person-centred approach when supporting Participants in their own home and/or out in the community.

Have a nursing background and/or are experienced in providing support to participants with intellectual and physical disabilities as well as mental illnesses and but also a willingness and ability to work with a variety of NDIS Participants with different levels of abilities.

Essential Criteria:

Experience in aged care, community care, disability care experienced with mental health.

Able to provide personal care to both male and female participants such as showering, grooming, toileting and continence or willingness to learn.

Experience supporting Participants using frames, wheelchairs, and hoists.

Experience supporting Participants with daily living skills, food preparation and housekeeping.

Must be willing to travel in Newcastle and surrounding areas.

Strong written and oral communication skills.

Must own a reliable vehicle with full comprehensive insurance.

Must be Covid-19 Vaccinated.

Licence/Certification:

NSW/International Driver’s Licence.

Current First Aid and CPR certificate.

NSW Working with Children’s Check.

NDIS Worker Screening Check.

NDIS Orientation Certificate.

New Worker NDIS Induction Module Certificate.

COVID-19 Training.

Must be vaccinated for Covid-19.

Pay is based on the SCHADS Award (Social, Community, Home Care & Disability Services Industry Award).
