    Colbrow Care provides premium support services for older Australians receiving Aged Care at home, children and adults who have physical, sensory and/or intellectual disabilities, as well as children and adults injured in road traffic accidents and adults injured in the workplace.

    ABOUT THE ROLE 

    This is a full-time role based at our Head Office in Vermont. 

    Hours will be 9am-5pm Monday-Friday with some flexibility considerable for suitable candidates.  

    Free nearby parking available. 

    As a Community Case Manager RN you will incorporate existing and/or new processes into Care Plans for clients and provide advanced Nursing services which will allow them the option to live in the comfort of their own homes as independently as possible, for as long as possible. 

    You will collaborate with medical professionals including (but not limited to) our team of Community Case Manager RNs, Client Care Coordinators and field staff including PCAs, EENs & RNs to assist in delivering high-quality, tailored care to clients. 

    Key responsibilities will include but are not limited to the following… 

    • Provide thorough 1:1 assessments of clients within their homes and formulate tailored Care Plans to enable field staff the ability to deliver safe, effective care and nursing services. 
    • Manage client queries and concerns in collaboration with the respective Manager. 
    • Contribute towards a culture of high-quality care through demonstration of leadership including but not limited to identification and management of hazards in the home. 
    • Communicate ideas on complex and sensitive issues clearly in writing, selecting language, tone and format to suit target recipients. 
    • Identify areas for procedural improvement and provide coaching and/or training to internal staff and/or stakeholders as needed. 
    • Build rapport with a wide range of interest groups, with differing agendas and values, developing and maintaining good working relationships for the longer term.
    • Attend infrequent, one-off call outs during work hours to assist particular clients.

    REQUIREMENTS 

    To be considered for this role, you must have the following credentials: 

    • Bachelor of Nursing 
    • Current AHPRA Registration
    • A minimum of 5 Years’ experience in a clinical setting 
    • Knowledge and understanding of current legislation in the Health Care industry and accreditation requirements 
    • A working understanding of typical roles and responsibilities within Aged Care and Home Care

    You will also have AND/OR be willing to obtain the following:

    • Current NDIS Worker Screening Check 
    • Current Working with Children’s Check
    • Current First Aid Certificate (HLTAID011) and CPR (HLTAID009)
    • Current Victorian Drivers License and reliable transport 
    • Valid working rights in Australia 
    • Mandatory competencies (as required by Colbrow Care)
