About the role:

You will manage a portfolio of community-based customers (with home-care packages) around the Perth metro area (particularly eastern suburbs). The key responsibilities include building relationships with elderly customers and their families, developing tailored care plans, managing budgets, and coordinating care services to maintain our customer’s independence. You will be inspired and motivated to make a positive difference to the families and achieving ultimate customer satisfaction.

Your strong networking skills will enable you to build rewarding external relationships within the relevant communities. Your week will be spent mostly out on the road around Perth visiting customers in their homes, while the rest of your time will be spent liaising closely with our internal teams to ensure the smooth delivery of care services.

This is an autonomous role where you’ll have the ability to manage your own schedule and work from home when possible.

About you:

  • Previous case management experience especially Home-Care Packages.
  • Advanced knowledge of the Community/Aged Care system and funding model.
  • Strong coordination and organisation skills.
  • Exceptional customer service, negotiation and influencing skills.
  • Excellent ability to connect with multiple stakeholders and think commercially.
  • Experience using a customer database or online CRM tool.
  • A current valid driver’s license and reliable vehicle.
  • A clinical background (EN, RN, allied health, social work, dietician, etc) would be great but not essential.
