Job Description

  • Central Adelaide Local Health Network – Mental Health Services
  • Total Indicative Remuneration AHP-3 $108,448/$116,253 p.a.
  • Total Indicative Remuneration RN-3 $123,346/$128,860 p.a.
  • Temporary from 13/2/21 up to 30/6/22 and part-time working 15 hours per week
  • The Way Back Support Service [TWBSS] is a non-clinical support service commissioned by the Adelaide Primary Health Network [APHN] and delivered by a selected service provider. TWBSS is focused on providing practical psychosocial support to people experiencing a suicidal crisis or who have attempted suicide. Its objectives are to,

  • improve access to high-quality aftercare to support at risk individuals to stay safe
  • build capacity of individuals to self-manage distress and improve mental wellbeing;
  • improve links with clinical and community-based services to meet individual needs and circumstances;
  • increase social connectedness and links to supportive networks (families, friends, peers and carers); and
  • improve the capacity and capability of TWBSS workforce to support at risk individuals.
  • The Way Back Support Service Support Coordinator is a pivotal liaison point between the MHCP, APHN and the service provider. This includes,

  • providing advice, consultancy and supervision to support coordinators.
  • managing clinical risks and incident responses
  • ensuring compliance with clinical governance requirements.
  • fostering positive relationships with community mental health teams as the primary referrer
  • being a crucial contact point for advice and escalation of care when needed
  • supporting the development of systems and processes to support referral, consultation and escalation.
  • maintaining effective relationships with the APHN and Service Provider for the purpose of promoting and referral to TWBSS
  • monitoring fidelity to TWBSS model of care
