You will be responsible for coordinating the funding outcomes by working largely with internal stakeholders to ensure congruency between the care provided to our residents, the residents care plans and the funding claimed.This role will cover our care homes in Adelaide.DutiesCoordinate and compile funding assessments and submissions to ensure claim congruency, accuracy, funding maximization and compliance with regulationsEnsure all submissions are appropriately supported with the correct evidenceEnsure documentation and charting is commenced for residents due for ACFI submission – voluntary and mandatory submissions – using the 30-day and 14-day planners as a guidePartner with the Clinical Care Manager and Registered Nurses to coordinate and monitor the completion of resident documentation, assessments and chartingLiaise with the Clinical Care Manager at least weekly to ensure assessments/ Care Plans and required documentation/charting is completed as appropriate and reflects care needsLiaise with Clinical Care Manager to obtain and follow up diagnosis required for ACFI submissionAssisting in compiling medication charts and ACFI packsPopulate the monthly ACFI template for voluntary upliftsConsult with the Funding Consultant, General Manager and Clinical Care Manager review ACFI submitted claims against the ACFI claim on the Payment Statements and appropriately action all funding variancesActively engage with and monitor resident’s and their care needsUndertake periodic reviews in consultation with the Homes Management team to ensure claimed care needs are being appropriately deliveredSkills and ExperienceRegistration with the Australian Health Practitioner Regulation Agency (desirable)Knowledge of the Aged Care Funding Instrument (ACFI) (essential)Sound proven written and communication skillsExcellent coordination skills and experience in leading processesSound administrative skills and ability to multitaskAnalytical skills (for Payment Statements and report analysis)Clinical Assessment Skills (desirable)Skills delivering education with guidance (desirable)CultureThis is a supportive and collaborative team, where you will work autonomously in your role.
