Job Description

Reporting into the Personal Support Coordinator will have a vital role to plan and deliver innovative and inclusive personal support and lifestyle programs for all residents to support them to live their BEST life.PST members assist residents by providing tailored individual holistic care and are respectful to the persons choices. Our PST program promotes independence and recognises the importance of each resident’s sense of self, promoting links to their community and social engagement.The PST frequently engage with residents and their families to discover and determine goals, assist in achieving and assess the outcomes enabling an individual to live as well as they can.Current duties being undertaken by PST members: 1:1 Goal identification and success planningLiaising with family for resident item needs i.e. additional clothingAssisting with internal scheduled exercise and/or therapy programsProviding transport for individuals as directed and agreed by the RSMTo engage with, on behalf of residents, appropriate community based individual or group activities for participationBasic skills and abilities assessment during activities3-5 Program aimed at clinically targeted residents for the purpose of relaxation and supportProvide assistance to our residents to maintain old and develop new relationships with family and friendsActing as an advocate for each individual About you:You are passionate about what you do and want to make a difference in the lives of our residents/clients, to support them to live life to their full potential.This is the place for you if you have: Certificate IV in lifestyle and leisure or equivalent or relevant industry experienceA current First Aid certificatePrevious experience coordinating a lifestyle program and lifestyle teams in residential aged careExperience driving a 12 seater busThrive in a team and also appreciate the one to one interactionsCommitment to being part of a workplace culture based on the BEST way of life (Believe, Excel, Support and Trust)Are willing to work within the ethos of the Uniting Church You will need: Registration to Work with Vulnerable People (TAS)Full Australian working rightsA current Police Records Check or be willing to undertake this processA current flu vaccination and be prepared to meet this annual requirement About Us:At Uniting AgeWell we promote and support our people to be their BEST and give their BEST so they can help our communities to live well with choice and peace of mind.
